Survive the Z is an immersive outdoor zombie experience involving physical activity, uneven terrain, low-light environments, fog effects, strobe lighting, loud noises, and simulated horror scenarios. Participation carries inherent risks including but not limited to slips, falls, and elevated stress responses.

By signing below, I confirm that I am the legal guardian of the named participant and that I authorize their participation in the Survive the Z event on 13 June 2026 in Woerden, the Netherlands. I acknowledge the risks described and accept responsibility for the participant's wellbeing.

Physical risks. The event involves running, crawling, navigating obstacles in low-light conditions, exposure to fog machines, strobe lights, and sudden loud sounds. The participant may experience elevated heart rate, stress responses, and minor physical contact from actors within the event's safety guidelines.
Medical conditions. I confirm that the participant is in adequate physical and mental health to participate. I have disclosed any relevant medical conditions, allergies, or medications to the event organizers. I authorize emergency medical treatment if required during the event.
Turning mechanics. Participants may be "turned" (eliminated) during gameplay, which may involve being tagged, startled, or directed by actors. The participant understands and accepts these game mechanics.
Photography and media. I grant Survive the Z and its affiliates permission to photograph, film, and record the participant during the event, and to use such media for promotional purposes without compensation.
Right to refuse entry. Survive the Z reserves the right to refuse entry or remove any participant who appears to be under the influence of alcohol or drugs, behaves aggressively, or fails to follow safety instructions from staff.
Validity. This authorization is valid solely for the Survive the Z event on 13 June 2026 in Woerden, the Netherlands.
Data retention (GDPR). Personal data collected through this form is processed for the sole purpose of event authorization and safety. Data is stored securely and retained for a maximum of 90 days after the event date, after which it is permanently deleted. You may request access, correction, or deletion of your data at any time by contacting info@survivethez.com.
